Feeling tired, overwhelmed, or isolated because of your chronic disease? The Chronic Disease PATH program gives you the tools to feel better. You will learn how to:
Deal with frustration, fatigue, pain, and difficult emotions
Be active for better health
Make healthier food choices
Talk with family, friends, and healthcare providers about your health
Program benefits may include:
Increased physical activity which can lead to better energy levels
Improved ways to talk to healthcare providers and family
Better quality of life
This workshop is free for Michigan residents. Family members, friends, and caregivers are encouraged to attend with you.

Workshop Details:

- The online workshop meets for 2 hours, once a week for 6 weeks. This is a live class using Zoom and it will not be recorded.
- To attend this workshop, you must have 1) an internet or data connection and 2) a device with a camera and audio.
- When you register, you agree to use your video and audio during the workshop and attend all six sessions. Please let your facilitators know if you are going to be absent.
