About
Feeling tired, overwhelmed, or isolated because of your chronic disease? The Chronic Disease PATH program gives you the tools to feel better. You will learn how to:
- Deal with frustration, fatigue, pain, and difficult emotions
- Be active for better health
- Make healthier food choices
- Talk with family, friends, and healthcare providers about your health
Program benefits may include:
- Increased physical activity which can lead to better energy levels
- Improved ways to talk to healthcare providers and family
- Better quality of life
This workshop is free for Michigan residents. Family members, friends, and caregivers are encouraged to attend with you.
The workshop meets for 1 hour, once a week for 6 weeks. You must have a telephone or computer to attend this workshop. When you register, you agree to attend all six sessions. Please let your facilitator know if you are going to be absent.
